Breast Augmentation Surgeries

Breast augmentation surgery is one of the most common surgeries in the world and in our country. Methods used for breast augmentations are prosthesis placement and fat injections. It is possible to enlarge the breast tissue by injecting the fat, taken from another part of the body, into the breast tissue by preparing it in the form of a stem cell. But, breast is a very special tissue which requires follow-up. Therefore, fat injection should be used in limited and rare cases.

Breast prosthesis should not only be used for breast enlargement. A prosthesis which is placed in a sagging breast does not recover the sagging and even it causes further sagging with the effect of weight. Therefore, depending on the degree of sagging of the breasts, silicone prosthesis should be applied together with breast lift surgery.

What are the types of breast augmentation prostheses?

Breast prostheses are categorized according to the filler materials, shapes and the structure of the outer layer. The common point in all of them is that the outer layers are made of silicone. According to the filler materials inside, prostheses might be filled with silicone gel (solid), saline or both (saline in the form of silicone gel around a physiological core).

The outer surfaces of silicone prostheses may be smooth or scabrous. The purpose for the scabrous surface is to ensure that the capsule structure which is formed around silicone prosthesis gets thin over time.

What are prostheses shapes?

Prostheses can be anatomical (like a water drop), round or asymmetric in shape. We decide which prosthesis to be used according to the shape of the breast (degree of upper fullness), the height-weight ratio of the person, the size of the chest and shoulder width, and the patient’s expectation. Anatomical (drop-shaped prostheses) provide a more natural image, and upper fullness is less than round-shaped types.

In which region can breast augmentation prostheses be placed with a surgical incision?

Prostheses can be placed through a 4 cm incision from the tip of the colored part on the nipple, folding area underneath the breast and axilla.

Nipple incision: If the size of the colored part of the nipple is enough for the prosthesis to pass through, the prosthesis can be placed by making an incision in semilunar shape from the junction line of the nipple and skin. However, there may be numbness in the nipple for up to 1 year. Another disadvantage is that the breast tissue is also incised.

Axilla incision: Breast and muscle tissue is reached by making an incision in the inner border of the hairy area in the axilla and prosthesis is placed. In this case, the surgical site becomes too far, it is a disadvantage especially in prosthesis placed under the muscle, because the most interior part of the muscle cannot be seen clearly. For this reason, it is more convenient to perform this operation endoscopically.

Incision under breast: It is possible to place prosthesis by making a 4 cm incision from the folding area underneath the incision. The scar is hidden under the folding area. The prosthesis can be reached without making an incision on the breast tissue for the prosthesis placed under the muscle. In this method, the surgeon can see the surgical site most comfortably.

How is the size decided?

The solution to this difficult issue is to fully understand the patient’s expectations. It is necessary to embody the size in the patient’s imagination.  Breast size and bra sizes are expressed as A, B, C, D ‘cup’. 75,80 refers to the circumference of the chest passing under the breast. After breast augmentation with breast prosthesis, your cup size will increase. In a normal and natural breast, the upper fullness of the breast begins at the 2nd rib level and the outer limit of the breast should not exceed the anterior axilla line. No matter how large prosthesis the patient wants, an appearance exceeding these limits is not aesthetic and should not be done.

Is it possible for a person to breastfeed after having undergone breast augmentation with silicone prosthesis?

Yes, she can breastfeed. Since prosthesis is placed under mammary glands and ducts in any case, damaging mammary glands and ducts is not possible.

Does breast augmentation cause cancer?

Breast prostheses have been repeatedly proven not to cause cancer and have been approved by the FDA.

How long does the operation last?

The operation lasts about 1.5 hours. It is performed with operating conditions under general anaesthesia. It is enough for the patient to stay in the hospital for 1 night. The dressing is removed on the 3rd day. It is enough to wear a sports bra for 6 weeks. The patient can return to work on the 5th day. It takes 2 months for breasts to gain the final shape.
